1 INT. TX HOUSE - DAY 1 We hear music as LUCY (10, Hispanic) rushes down a set of stairs. Greeting Lucy at the bottom of the staircase is her mom, VANESSA (30’s, Hispanic). Vanessa is not only a wife and mother, she is a BCBSTX clinician for HealthSelect participants. Vanessa gives Lucy her backpack for school as she reaches the bottom of the staircase. They both walk out of the house using the front door. 2 EXT. TX HOUSE - DAY 2 Vanessa follows Lucy out of the front door. She waves to Lucy who rushes to the sidewalk where she begins her walk to school. Vanessa walks as she talks into camera. VANESSA As a care management clinician, I provide personal support to HealthSelect participants with ongoing health conditions... 3 EXT. TX OFFICE - OUTSIDE OF MAIN ENTRANCE - DAY 3 Vanessa walks into her office building. She walks as she talks into the camera. VANESSA Want a peek into what all this means? Let me tell you about Oscar, who I had the pleasure of helping out this past year. 6 EXT. IL HOUSE - DINING ROOM - DAY 7 We see Oscar (late-40’s, African-American) sitting at his dining table with his teenage son, KEENAN (14, African-American). Oscar looks sad. We hear Vanessa’s voice over. VANESSA (V.O.) Oscar is 48 years old and has Type 2 diabetes. He is recently divorced and living with his son Keenan. Keenan barely touches his food. He looks sad. We hear Vanessa’s voice over. VANESSA (V.O.) (cont'd) Oscar had a question about his benefits so he called the 1-800 number on the back of his medical ID card to speak with a BCBSTX Personal Health Assistant. 8 INT. IL HOUSE - LIVING ROOM - DAY 8 Oscar picks up his phone and calls BCBSTX. We see him talk, but don’t hear his voice. We hear Vanessa’s voice over. VANESSA (V.O.) In talking with Oscar, the Personal Health Assistant learned he had been experiencing stress and could benefit from talking with a clinician. She transferred Oscar to me. 2. VANESSA I pulled his history of doctor’s visits, lab results and medications. This allowed me to have a basic understanding of his situation. 10 INT. IL HOUSE - LIVING ROOM - DAY 10 Oscar is on the phone with Vanessa. He talks, but we can’t hear him. We hear Vanessa’s voice over. VANESSA (V.O.) Through speaking with him, I also learned that he recently went through a divorce, which caused him and his son a lot of stress. 11 INT. TX OFFICE - CUBICLE - DAY 11 At her desk, Vanessa talks into camera. VANESSA I also found out that he had a history of uncontrolled blood sugars and had not seen an endocrinologist in over a year. 12 INT. IL OFFICE - CUBICLE - DAY 12 We see Oscar at his desk at work. He looks stressed as he eats a burger and fries at his desk. We hear Vanessa’s voice over. VANESSA (V.O.) He was trying to maintain her blood sugars through diet alone. This was a struggle for him and most of his meals consisted of fast food and sugary snacks. 13 INT. TX OFFICE - CUBICLE - DAY 13 At her desk, Vanessa talks into camera. 3. (Printed with an unregistered version of Fade In) VANESSA He sounded discouraged but I told him not to worry too much because I was there to help walk him through a plan that could help him. I told him that it was important to see his endocrinologist at least every 6 months and he agreed to schedule an appointment right away. 14 INT. IL HOUSE - LIVING ROOM - DAY 14 Oscar is on the phone with Vanessa. He talks, but we can’t hear him. We hear Vanessa’s voice over. VANESSA (V.O.) I told him stress can have an impact on blood sugars and that we could get him help to manage the stress from his divorce. With his permission I transferred him to a mental health provider. 15 INT. TX OFFICE - CUBICLE - DAY 15 At her desk, Vanessa talks into camera. VANESSA Before I transferred him over, I made sure we also had a follow-up call scheduled for a week later. We definitely got a lot done on that first call, but every participant is different. Some are like Oscar, while others need a little gentle coaxing. 16 INT. IL GYM - DAY 16 We see Oscar. He is walking on a treadmill. He looks happy and motivated. Close up on Oscar as he smiles and walks on the treadmill. We hear Vanessa’s voice over. VANESSA During our follow-up call, it was amazing what a difference a few weeks made! Oscar sounded happier, hopeful and motivated to take control of his health. 4. (Printed with an unregistered version of Fade In) 17 INT. IL OUTSIDE OF GYM DOORS - DAY 17 After the gym, Oscar is on the phone with Vanessa. He is smiling as we hear Vanessa’s voice over. VANESSA (V.O.) He said the call with the mental health provider went great and that he will continue regular sessions to help him cope with his stress. 18 INT. IL DOCTOR'S OFFICE - DAY 18 We see Oscar at his appointment with DR. ADAMS (40’s, Caucasian), his endocrinologist. We hear Vanessa’s voice over. VANESSA (V.O.) He even went to his first endocrinologist appointment where he had lab work completed. 19 INT. IL DIABETES EDUCATOR'S OFFICE - DAY 19 We see Oscar meeting with ANGELA (30’s, Caucasian), a diabetes educator. We hear Vanessa’s voice over. VANESSA (V.O.) The endocrinologist scheduled him to meet with a diabetes educator to discuss diet and blood sugar management. I was so proud of him for being so proactive! 20 INT. TX OFFICE - CUBICLE - DAY 20 In Vanessa’s office, we see her talking on the phone, but don’t hear her voice. We hear her voice over. VANESSA (V.O.) I told him about some educational videos about diabetes management, and he was happy to have me send them to him. 21 INT. IL HOUSE - LIVING ROOM - DAY 21 Oscar is on the phone with Vanessa. We scheduled another follow-up call and we continued to work together to keep him on the right track. 22 INT. TX OFFICE - CUBICLE - DAY 22 In Vanessa’s office, we see her talking on the phone. She looks into camera. VANESSA How did things end up for Oscar? Well, see for yourself. 23 INT. IL HOUSE - KITCHEN - DUSK 23 We see Oscar in his kitchen. He is cooking a healthy supper with Keenan, his teenage son. They are smiling and happy. While he cooks with Keenan, Oscar talks to camera. OSCAR It’s been four months since that first phone call with Vanessa and I feel so much better. I had no idea this type of program was even available to me. Oscar moves around the kitchen as he prepares supper. We see close up shots of the food he is preparing as we hear his voice over. OSCAR (V.O.) I felt so lost and discouraged trying to manage my health on top of everyday life. Her support made it easier and she guided me every step of the way. Oscar talks to camera again. OSCAR Having those follow-up calls held me accountable. I also didn’t want to let Vanessa down because I could see how much she cared and how hard she was rooting for me to make changes in my life. IL HOUSE - DINING ROOM - DUSK 24 Oscar looks into camera as he places a plated meal in front of Keenan who is seated at the dining table. OSCAR I know I’ll have my ups and downs with my health, but now I know I have a whole support system. That means more to me than you could imagine. The camera pulls back to reveal Oscar and Keenan eating dinner. They smile and talk, but we don’t hear them. We hear uplifting music to represent their joy. 25 INT. TX OFFICE - CUBICLE - DAY 25 We see Vanessa in her office. She is leaving for the day. She stands outside of her desk and looks into camera. VANESSA Regardless of your health struggles, we are here to help. If you think you can benefit from a care management clinician, call a BCBSTX Personal Health Assistant and ask to speak with a clinician. 26 INT. TX OFFICE - HALLWAY - DAY 26 Vanessa walks down a hallway and looks into camera. VANESSA We help people with all types of health concerns such as heart, high blood pressure, musculoskeletal and asthma, to name just a few. We work with your physicians to assist with following your treatment plan. We can also help arrange appointments, coordinate transportation to and from appointments, ... 27 EXT. TX OFFICE - OUTSIDE OF FRONT ENTRANCE - DAY 27 Vanessa walks out of the building and looks into camera as she walks. 7.
